Executive Summary
A research team led by Stanford Medicine partnered with Onetree to address a growing challenge in medical research: the decline in survey response rates and the increasing difficulty of engaging participants over time.
To tackle this problem, our team designed and developed HIPAA-compliant mobile applications for iOS and Android, focused on simplifying data collection and improving the participant experience. In parallel, we built monitoring dashboards that enabled researchers to track response rates, subscriptions, and engagement metrics, while gaining deeper visibility into participation patterns through paradata and data visualizations.
This collaborative, cross-disciplinary initiative brought together researchers, epidemiologists, statisticians, healthcare professionals, and survey experts. By aligning diverse stakeholder needs into a cohesive product experience, the project helped increase survey participation, improve engagement through mobile-first interactions, and support researchers with actionable insights throughout the data collection process.
The Challenge: Increasing survey participation while ensuring data quality and compliance in medical research
Medical research teams were facing a persistent challenge: declining survey response rates and growing difficulty in sustaining participant engagement over time. Traditional data collection methods were becoming less effective, costly, and harder to scale, limiting the quality and continuity of research insights.
At the same time, any new digital solution needed to meet strict privacy and security requirements, including full compliance with HIPAA regulations, while remaining easy and accessible for participants with varying levels of digital literacy.
Beyond participant engagement, researchers also lacked real-time visibility into response behavior. Understanding when, how, and why participants dropped off, and how different recruitment and notification strategies performed, required better tools to monitor engagement, analyze paradata, and make informed decisions throughout the data collection process.
This combination of engagement challenges, regulatory constraints, and limited research visibility defined a complex problem space that demanded a carefully designed, user-centered solution.

The Solution: Mobile-first data collection designed for engagement, insight, and research needs
To address the challenges of participation, visibility, and compliance, our team designed and deployed two mobile applications for iOS and Android, purpose-built to support medical research data collection while prioritizing ease of use for participants and actionable insights for researchers.
From a participant perspective, the apps focused on simplifying survey completion and encouraging continued engagement over time. Features such as push notifications, clear interaction flows, and gamification elements were incorporated to motivate repeat responses and reduce drop-off throughout the research process.
In parallel, we developed two monitoring dashboards tailored to researchers’ needs. One dashboard provided visibility into response rates, subscriptions, and engagement metrics, while the second, developed in collaboration with Meta, enabled teams to evaluate recruitment performance and advertising investment, linking acquisition strategies to app downloads and participation outcomes.
All solutions were designed to meet HIPAA compliance requirements, ensuring the privacy, security, and integrity of sensitive health data. Additionally, the dashboards integrated data from multiple sources, including analytics platforms, backend systems, and external APIs, allowing researchers to analyze paradata, identify participation patterns, and make informed decisions throughout the data collection lifecycle.

Results & Impact: Improving participation efficiency and reducing research recruitment costs
The implementation of mobile-first data collection and monitoring tools delivered measurable improvements across both participation outcomes and operational efficiency.
By leveraging social media–driven recruitment and mobile survey experiences, the project significantly reduced acquisition costs compared to traditional methods. The cost of app downloads was four times lower when using social networks versus conventional ABS approaches, while participant recruitment through social media tripled relative to traditional channels.
From a research engagement perspective, the mobile applications helped address the core challenge of declining response rates. Survey participation doubled compared to previously measured methods, demonstrating the impact of simplified mobile interactions, timely notifications, and engagement-driven design.
Beyond participation metrics, the monitoring dashboards provided researchers with greater visibility into response behavior and recruitment performance, enabling informed comparisons between digital and traditional data collection strategies. This combination of improved engagement and clearer insight helped validate mobile-first approaches as a scalable and cost-effective solution for medical research data collection.
